What to Expect on Your First Visit

    Your first visit at Transcend Physical Therapy is a chance for you to meet your therapist, gain information on your condition, and learn what your customized treatment plan will involve. Your therapist will discuss with you the specifics of your injury and perform a comprehensive evaluation including various tests and measurements. The evaluation may include assessment of your strength, range of motion, flexibility, pain level, balance, gait, endurance, and functional limitations. The information gathered by your therapist with assist in identifying the cause of your injury and guide your treatment plan.

How to get the most out of your first visit:

    Arrive at least 15 minutes prior to your scheduled appointment to allow time for you to fill out the necessary paper work, or, better yet, download and complete the required forms from our download section which are located on the right of this page.

    Wear loose-fitting comfortable clothes and tennis shoes

    Bring the following items:
          Your referral/prescription for physical therapy
          Current Insurance cards
          Driver license/identification card
          A list of all medications
          A list of all medical conditions and past medical and surgical history
          Any imaging results (x-rays, MRI, etc) that are related to your condition

Your Rights and Responsibilities as a Patient

    As a patient of Transcend Physical Therapy, you will have several rights and responsibilities. You have the right to privacy as described by the Health Insurance Portability and Accountability Act (HIPAA). You have the right to receive the best possible treatment of your condition and if at any time you are unhappy with the care you are receiving, it is imperative that you bring this to the attention of your therapist or Transcend Physical Therapy management. Finally, to ensure the best outcomes of treatment, you have the responsibility to remain compliant with the treatment agreed upon by you and your physical therapist, including recommended attendance of treatment sessions and performance of home exercise programs which are designed to expedite your recovery.
